This form is used to submit all the data required to create a service request. It can be used by Ottobock subsidiaries that do not have SAP access for the following issues: complaints, scheduled maintenance, stock returns, quantitative delivery errors, technical updates or modifications, and repairs without warranty.
This is the last form we need (in English and German) for SAP-OBAs that create the notification directly in SAP and which should be available via a link in SAP. In this form we need the "copy to clipboard" button (like Ctrl+A and Ctrl+C) to copy the questions and chosen answers of the form and directly paste them into a SAP text field via Ctrl+V.
