This form is used to submit all the data required to create a service request. It can be used by Ottobock subsidiaries that do not have SAP access for the following issues: complaints, scheduled maintenance, stock returns, quantitative delivery errors, technical updates or modifications, and repairs without warranty.
This is the form we need only in English for SAP-OBAs when the customer is filling the form directly (in the overview customer data2 and PSQ 2)
