Ottobock Returns Process

As our commitment in supporting you, your patients, and your business, we have simplified the returns process as there are two options to choose from when it comes to returns.

For faster service

Option 1

Complete the form below (including all mandatory information) and upload photos of the product intended for return. Be sure to include the following photos:

  1. Image of serial number or lot number

  2. Image of product number

  3. Image of any damage for a warranty request

  4. Any additional images that will help determine the issue at-hand.

A Customer Service Representative will follow up with you regarding the return details. Unless otherwise requested, there is no need to the ship the product intended for return.

Option 2

Contact Customer Service at +1 (800) 328-4058 to obtain a Return Authorization Number (R.A. #). This is valid for 30 days.

Please follow the instructions below to complete the process:

  1. For lithium-ion batteries and products containing them, read the shipping and return instructions carefully (provided below).

  2. Display your RA number prominently on the outside of the packing box. Your RA number can also be found in your Returns list.

  3. All returns and paperwork should be sent to the following address:

Please Note: Issuing of an R.A. is not a guarantee to provide credit or replacement. Decisions are made only after product is evaluated.
